Transcript Requests

Please Note:

• Current students request transcripts through Naviance, not the form below. 
• Unofficial transcripts will only be given to the graduate. 
• Official transcripts will be sent to a University, Recruiter or Employer only. 
• Transcript requests could take up to 5 business days to complete.

Only former students may use this form to request a transcript. All other inquiries, including education verifications, should be sent to transcript@mtsd.org

Graduates please use this fillable transcript request form, fill it out with a copy of your photo ID (Government issued driver’s license or passport). Email  transcript@mtsd.org if you have questions:
